Location Information
The name of the state or province where the encounter took place.
The name of the county where the encounter took place.
The name of the closest town to where the encounter took place.
The name of the nearest road or major road crossing to where the encounter took place.
Please supply any additional pertinent information about the sighting location.
Choose one or more terrain types that best describe the sighting location.

Step 4 of 8: Date & Time

The date when the sighting occurred.
Enter the approximate time of the sighting (HH:MM).
Please indicate the season in which the sighting occurred.
Please indicate the general time when the sighting occurred.

Step 5 of 8: Weather Conditions

Degrees Fahrenheit.
Please indicate the general sky conditions present when the sighting began.
Please indicate any precipitation present when the sighting began.
Please indicate the wind when the sighting began.
Please indicate the predominant wind direction when the sighting began.

Step 6 of 8: Creature Details

Estimate the height of the creature or creatures to the nearest half foot.
The estimated weight of the creature or creatures in pounds.
Please indicate the number and apparent sex of each observed creature.
The predominant hair color observed on the creature or creatures.
Estimate the average length of the hair in inches.
If observed during the day, describe the eye color.
If observed at night, describe the eye color.
The distance of the creature from your viewpoint in yards.

Step 7 of 8: General Encounter Information

Please indicate how many male were present.
Please indicate how many females were present.
Please indicate how many under 18 females were present.
Please indicate how many under 18 males were present.
Describe the creature's behaviors during the encounter.
Please describe the specific human behaviors immediately before and during the encounter.
Please re-tell the event starting with what you were doing immediately before you observed the Sasquatch.
Additional pertinent information related to the encounter.

Step 8 of 8: Evidence

Some people benefit by revisiting the location to search for hair or footprints. Others capture photographic or audio evidence at the time of the encounter. If you have obtained any evidence, the Olympic Project would be interested in conducting further analysis, with your permission.

Please indicate whether a hair sample was collected. If so, the Olympic Project may request a sample for study.
Please indicate whether an audio recording was collected during the encounter.
Please indicate whether a cast of one or more footprints was obtained.
Please indicate whether any photos, video, or film evidence was obtained.
